NDIS Support Coordination
Support Coordination is about helping you understand, navigate, and make the most of your NDIS plan. It involves working alongside you to connect with services, build your confidence in coordinating supports, and overcome barriers that may make accessing the right assistance difficult.
What will support coordination be like?
We aim to work with you in a collaborative and person-centred way, that is guided by your goals, values, and lived experience. We recognise that navigating service systems can be overwhelming, particularly when you're also managing disability, mental health challenges, trauma, or systemic barriers.
We see Support Coordination as a relational role, not simply coordinating services, but understanding the broader context of your life and helping your supports work together in a way that is meaningful, sustainable, and responsive to your changing needs.
We will help you:
Build the right team around you - We can help you connect with and advocate for the supports you need, including allied health professionals, medical providers, mental health services, community supports, and other NDIS providers.
Keep everyone on the same page - With your consent, we help facilitate clear and consistent communication between your support team so everyone is working towards the same goals and you don't have to repeat your story to every provider.
Supporting you in a way that works for you - Disability is dynamic and individual, and there's no one-size-fits-all approach. We work flexibly, providing accommodations where needed and helping you develop practical strategies that build on your strengths, reduce barriers, and make everyday life more manageable.
Advocacy and Navigating Change - The NDIS can be complex, and your support needs may evolve over time. We can help you understand your plan, prepare for reviews, respond to changing circumstances, and advocate alongside you when needed. Our aim is to help you feel informed, supported, and confident when making decisions about your supports and future goals.
